Branson 3820 rear axle issue

I am a mechanic at a Kioti dealer in Indiana. Customer brought in a 3820 that the right rear axle was working itself out. Upon disassembly, I noticed that the final drive had been removed before. I disassembled and found the three axle retainer bolts broke. Two had the heads popped off and the third was sheared off flush with the axle. I discovered from the owner that the same expect failure happened in 2018. Has anyone else experienced this failure? Yeah, I could put it back together and it will probably last him another 5/6 years but I would rather determine the cause and fix it properly. Thanks in advance for any replies

Is it possible the operator is activating the differential accidentally, intentionally but improperly, is it geaR drive?
 
Yes, it is a gear drive. I thought of that but usually when the diff lock is engaged while spinning, the spider gears or the pins that lock them together are what lets loose. These bolts are basically just keepers that hold the axle from coming out. The more I have looked at it, I believe the right rear is rapidly spinning in reverse then catching traction. When this happens, the bull gear would be side loaded heavily and could possibly pop the heads off of the keeper bolts. This is just my speculation of course
