I just don't know what to do next.

Do you have spacers enough under it that you have to push down to get the tester ears locked on the neck and it tightens with some force required? Pressure of the spring against the spacers is all that is making the seal hold.
Yes sir, I have a good seal there, Let me try again and plug the overflow tube. forgot that.
 
You do not want to seal around the cap. The seal MUST be at the step (seat) that the pressure valve on the cap sits on. Is not necessary to plug the overflow nipple.

If the cap is loose, it is not pressing on the relief valve seat. There should be rubber spacers in the kit that are dropped down the neck to make the test cap tight on the neck.
 
I guess I am just not getting a seal around the cap, I have tried several different gasket material and thickness. Not sure what to do at this point.
I haven't had hands on a Maddox tester. It looks like my Stant. Does the pumped air come out the center as I marked? The thick washer with my tester matches the OD and ID of the seal washer the arrow points to. The cap requires a good bit of force to lock it on the filler neck with the washer in place. If you are using multiple rubber pieces you may need to glue them together to make a solid spacer to use under the tester cap.

Yeah That is the same set up, I am thinking about trying a different one. I will fiddle with this one a little more, But if it still doesn't work I will try something different.
 
Way to small, Doesn't even begin to go on.
Maybe the other adapter kit will do it. The online manual for the kit reads like those expand like a boat drain plug. If that is the case, I have to guess they seal to the sides of the filler neck and cover the overflow tube opening. Again, I have never had hands on one of those.

I have the exact same kit (and as I wrote previously) it worked just fine on an oilallover radiator with a similar (if not IDENTICAL deep filler neck) with the addition of a rubber washer WITHOUT any adapters other than the one permanently attached to the hose from the pump.

I wonder if the O.P. is missing something as how the adapter latches to the filler neck, it takes a bit of finesse to do that, it attaches to the neck and also you need to turn the "wing nut-like" part of the adapter to tighten it up air-tight.

Could be, as I said mine is a Stant Kit, I have not had hands on the Maddox kits. But I would expect it would work, and you say yours did.
