Hi, I was wanting to install a block
heater on a MF 165 with the ad4203 diesel.
On the left side of engine there is a plug
in the center with a small threaded plug
located in the center of it and towards
the back of the engine on the left side
there is a freeze plug. In the attached
photo it's the one at the top right above
fuel line. Which plug would work the best
for a block heater? Thank you for the
help!

The in-block heaters tell you where they are designed to fit and the orientation the heating element must be in, if there is only one way it fits. The casting plugs, and pipe plugs, may look the same from the outside but open space behind them, inside the block, is often different. Find a heater and read the instructions for that heater as to where it installs. It looks like most go in a threaded hole on the left side of the block, so it may be the one you reference.

My late model 65 with the AD4-203 has the block heater on the right side.

The Zerostart Phillips/Temro catalog shows part # 3100093 fits the AD4.203. Shows it's a 3/4 pipe thread style for the block left side. The AD3.152 uses part # 3100013 freeze plug style on the block right side. O'Reilly's are Zerostart heater dealers.
