TO 30 exhaust connection

Friend of mine has a TO 30 and can't figure out the connection for the exhaust pipe off the manifold. I have not looked at it yet. He says the connection is never tight and continues to fall off. Any suggestions appreciated?

also make sure it is on correctly, as clamp get tightened it pulls exhaust/muffler to the manifold.
