    Chuck, yeah, I'm pretty sure it's a fleam. They're used for bleeding. I confess that I haven't seen one precisely that style but it makes sense to have a protective guard that when you squeeze the handle, exposes the blade. The blade should be fairly to 'good lord that's sharp'. Cool tool.
