Cole Planter ID

My neighbor bought the Cole planter pictured below and wants to know what model it is and if anyone has the seed spacing chart under the seed hopper lid. The chart is so faded and torn you can't see much of anything. I have called Cole planter company but no one ever answers the phone and so far hasn't called me back. Has anyone on here ever converted one like this to 3 point hitch? Thanks!!

Lots of info in this post, BTW the first cole planter I brought was mounted to a 3 point cultivator just like in the link in this post. This was a popular conversion to a rear three point cultivator. I converted mine to a fast hitch to fit my Farmall 100. In my hood where off set Farmalls are king fast hitch conversions were popular.

I just bought a couple old cole planters from an auction this weekend. One is mounted on an a 3 pt. A frame cultivator. The cultivator and the planter both have Cole decals on them, and the cultivator frame looks to be originally painted white and the planter green. It came with a 32 hole pea and soybean plate and a 8 hole hybrid corn flat. My question is the corn plate that I have the correct one for planting sweet corn? The other planter I bought is a double seed hopper style that looks like it will mount to the A frame cult. the same as the one thats on it already. It has two 6...
