188d in 530 bends push rods

My 530 with a 188d bends push rods on the 2nd cylinder from the front. It was like that when I got it, then bent both again. This time it broke the push rod on the valve towards the rear of the engine on that cylinder. The valve seems to be loose. I have had them adjusted to spec so they weren't loose or tight. What could cause that?
