JD Plow - Which Model?

Looking at a 4 bottom trailer plow w/ hydraulic lift to pull w/ my 730. Found one that a neighbor used with his 730 years ago, so I know I can handle it.

Question is: what model plow is this? Does not look like a 55 or 555, is it a 610H? 630H? Something else? How do I tell?

Also, I assume it should have a 12 inch rear tire on it. I want to obtain a parts book and operators manual so I can hook it up properly. Any help would be greatly appreciated.

H, the PLOW is a MODEL # 640,1960 & up as it has the INCREASED TRIP STANDARDS and has the STRAIGHT tail wheel LIFTING ROD. Also , the plow could have either 12" or 15" tail wheel, your preference but was 12" as orig equip.
