What is the proper hose connection on the Midwest WH-5 hoist. Is it just a straight hose threaded into the 1/2 FNPT end of cylinder or does there need to be an angle fitting? My hoist pinched/sheared the hose at the cylinder twice.
 
Well, it needs to be what fits your application.

I think I usually see a straight in hose, but that doesn’t mean every bolster and wagon frame will clear that exactly?

Could the cylinder be rotated 90 degrees and have the hose connection on the bottom. Looks like the front of the hoist should be notched, as to allow the hose to come through and not get sheared off.

I think you meant 180 degrees.

Seeing the box down, might shed some light on the problem. From looking at pictures on the web, the port on the cylinder looks to be in the correct location. The framing of the box appears more open in the pictures; this looks like the front box framing could trap the hose when it is fully down.
 
If those 2 red pieces meet when the hoist is down, that will never work.
I'd suggest putting a 45 out of the cylinder & go under the frame.
Option B would be to cut a hole in that bottom flange/ plate to run
the hose through.

I'd suggest putting a 45 out of the cylinder & go under the frame.
Seeing the photo of the hoist/bed lowered I think Brownie has your answer assuming that as I interpret the photo there is adequate room for a hose between the upper and lower sections of the hoist. The 45 elbow may not be turned directly down when installed only off to the side slightly to give the hose clearance. Also, this will only be the cure if the hoist/wagon bed and its frame are setting on solid stops of some sort like those of the hoist in the last reply by 4620BW in the link to this past YT thead. I’ve got a dump wagon with a Midwest hoist. I had to put a 90 deg fitting coming out of the cylinder coupled to another 90 deg fitting and then the hose. I had to use 2- 90s coming out of the cylinder to keep the hose out of harms way.
