Clutch assembly jumps off yoke. Is this caused by throw out bearing?

I can jimmy it back in. Then clutch works great again. Until it doesn't . :)

But yeah, guess I got to man up and tear it down.

Thanks.
You answered my next question. I wondered if the other side of the fork was broken off. I guess I'm on the side of thinking the tube is worn out so the bearing and carrier are dropping down.

Chiming in as I am also in the process of pulling the engine as of yesterday to get to the clutch area on my 1962 Oliver 770 gas. At first you could press the clutch and it would still drive but if you tapped the brakes or hit a bump it would disengage. Through the inspection cover I sprayed degreaser and brake clean and that fixed it. Shortly afterwards I was driving it and all of a sudden it started vibrating and making a racket then that suddenly stopped and so did my propulsion.
