I've had those used on my ears. I'd swear the tool was going to come out of my other ear when he got digging deep! Since then they've been using syringes and warm water on my ears. Sometimes they get a little carried away with the pressure from the syringe. Gets downright painful sometimes!

I was talking to a friend of mine a few weeks ago and he had the same experience. Said his ears hurt for a few days.
They used a little mini vac the last time I went.
A little loud obviously but not painful in the least. No pretreatment with water or softening agents and it worked great.
