Welcome to YT forums and ASAP parts. The most common issue with belt throwing is misalignment of the pulleys. this can be caused by a replacement pulley that is not the same offset as original, or poor design from the beginning. A worn belt is the second most common issue. when the belt wears, they can flip upside down, or at an angle causing them to climb the pulley flange and come off. The third is a loose belt or a tensioner that is not doing its job. Belts need to be snug, but not radically tight to stay on. There are also belt guides that are important to keep the belt in alignment especially if the belt is used as a clutch. Jim
